Birth Registration 

The province's Newborn Registration Service via the ServiceOntario website allows parents to electronically submit the Statement of Live Birth and applications for a birth certificate and social insurance number at the same time.

Death Registration

Death registrations are submitted to the municipal clerk's office by your chosen funeral director. The forms are then forwarded to the Office of the Registrar General for registration. Please note that death registration documents are not retained by the municipal office and any request for information must proceed through the Registrar's Office.

Obtain a Birth, Death, or Marriage Certificate

Ontario birth, death, or marriage certificates are ordered directly from ServiceOntario:

• Get or replace an Ontario birth certificate
• Get a copy of an Ontario death certificate
• Get a copy of an Ontario marriage certificate
